Instructions for Self Test

Step 1:
Collect Saliva Sample: » Saliva Sample Collection Instructions

Step 2:
Fill out the » Consent Form Human or » Consent Form Animal and return the sample and required fees to the address on the form.

Step 3:
Fill out » General Information Form

Step 4:
Send completed forms, saliva sample and check or money order payable to Dog Patch / Joyce Tasker for $295.00 for complete analysis.

The self test fee includes screening, products, shipping and handling and support.

Animal $295.00* (includes imprinted tincture)

Laser for Human or Animal add an additional $125.00*

* Self Test Screening Session fee must accompany sample and cost of laser
if you opt for a laser.
