Step 2:
Fill out the » Consent Form Human or » Consent Form Animal and return the sample and required fees to the address on the form.

• If you are *not* referred by a doctor or veterinarian, please sign and return
  the » Consent and Release Form Human or » Consent Form Animal

Step 3:
Fill out » General Information Form

Step 4:
Send completed forms, saliva sample and check or money order payable to Dog Patch for complete analysis.

The self test fee includes screening, products, shipping and handling and support.

Laser for Human or Animal add an additional $125.00*

* Self Test Screening Session fee must accompany sample and cost of laser
if you opt for a laser.

Pre-paid screenings are available.
3 screenings paid in advance per individual client $800.00 (savings of $85.00).
